疫情影響下流域城鎮韌性探討 : 以中台灣大安溪流域為例。

台灣流域相關災防研究多以硬式工程與傳統水資源管理為主,尤其探討流域中上游地區結構物的衝擊與損害風險,提及流域內之都市規劃與社會經濟、生態環境等層面較不重視,且流行疫病傳染影響下城鎮韌性表現更是著墨甚少。大安溪流域為為台灣中部地區重要產業、文化、民生聚集地區,流域境內產業形式多元,流域內具宗教文化中心(大甲鎮瀾宮)以及科技工廠地區(中部科學園區),除了盛產芋頭等雜糧農產外,還發展花卉、蔬果等精緻農產。2021年5月各台灣城市進入新冠疫情第三級警戒,延伸出有別於過往極端天氣衝擊下的公衛危機,如:無法負荷大量的醫療需求導致系統崩潰(system collapse);觀光人口銳減導致餐飲業歇業問

題。 本研究希望能了解疫情對於城鎮之間的衝擊影響,並透過空間分析方法以及指標評估來進行分析,了解流域城鎮在疫情衝擊的主要問題以及空間變化,本研究主要目的包括:(1).探討臺中市與苗栗縣交界之大安溪流域城鎮在疫情大流行背景下,城鎮衝擊影響,並對未來可能發生的公衛危機提出建議; (2).流域城鎮在不同地域空間(上游、中游、下游)具有其特點,不同城鎮的特點在災害衝擊下,是否會產生對城鎮造成加劇災害影響的脆弱性,亦或是會具有減緩與調適能力的韌性;(3).研究大安溪流域城鎮在疫情大流行前後韌性概念變化(過往自然災害衝擊與疫病公衛危機的不同),以及城鎮的韌性在流域空間中的差異性。研究方法分為量化方法與質

化方法並行,量化研究主要進行韌性的指標評估與空間分析,質化方法透過文獻分析、實地調查與訪談來補充以及驗證量化方法研究成果。透過田野調查訪談、韌性指標評估以及迴歸模型等方法,研究發現大安溪流域中的城鄉差異會導致疫情衝擊影響有所變化(例如,下游城鎮芋農比起中游城鎮的果農更容易受到疫情影響、上游偏鄉聚落的就醫與資訊取得的困難問題)。研究認為未來發生相同傳染性疫病造成的公衛危機時,透過韌性評估與空間分析方法鎖定重點城鎮,以達到減災與提升地區韌性之效果是可行的,透過全面性的評估與整合政府與利害關係人的調適能力與脆弱表徵,可以降低疫情帶來的衝擊影響以及做出因地制宜的城鎮規劃。

由於在肝細胞癌(hepatocellular carcinoma)的發展中缺乏特定的生物標記物以及其不明顯的早期症狀,導致肝細胞癌在診斷及治療方面的困難。在全球流行病學調查中,由代謝症候群(metabolic syndrome)所引起的肝細胞癌病例正在逐漸增加。許多研究已明確顯示,癌症發展中的代謝改變,與癌症的一些惡性表現型有很密切的關聯。YB-1 (Y-box binding protein 1)的多功能性已經在許多研究中證實,而在癌症發展中、包括肝細胞癌,YB-1扮演著致癌基因的角色。但YB-1是否會改變肝細胞癌中代謝的模式,則需要進一步的探討及驗證。本論文首先在諸多肝癌細胞株中,依據其

本身YB-1之表現量,分別挑選適當之細胞株,以建立 YB-1表現低下與YB-1過度表現的肝癌細胞株。接著以能量代謝測定實驗,發現在肝癌細胞株Hep3B、SK-Hep-1和PLC/PRF/5中提升YB-1表現時,會導致細胞於氧化磷酸化(OxPhos)中增加ATP的生產及使用;在肝癌細胞株Hep3B、SK-Hep-1以及HuH-7中降低YB-1表現時,則會有相反的結果。而在糖解作用(glycolysis)測定中,高表現YB-1的肝癌細胞株對於糖解作用沒有顯著性的影響,在低表現YB-1的肝癌細胞株中亦是如此,同時利用乳酸分析實驗(lactate production assay)再次驗證由糖解作用測

定中獲得的結果。因此,YB-1係與肝癌細胞的OxPhos代謝途徑改變有所關聯,而非糖解作用。此外,在許多文獻中指出,YB-1可藉由轉錄調控其下游基因的表現。在即時定量聚合酶鏈反應中,發現低YB-1表現量的肝癌細胞株Hep3B中數個氧化磷酸化相關基因的mRNA表現量有上升的趨勢;而在高YB-1表現量的Hep3B中,則沒有太顯著的差異,因此YB-1或許可能藉由其他機制造成肝細胞癌的代謝改變。先前研究顯示,當YB-1的表現量降低時,可抑制癌細胞之癌源幹細胞特性,包括自我更新以及遷移的能力。本論文發現低YB-1表現量確實會導致肝癌細胞Hep3B於自我更新及遷移能力的降低,而根據前述實驗所得到的結果,本

研究發現以氧化磷酸化抑制劑rotenone處理細胞後,則會減弱YB-1低表現量所帶來的效果。綜合前述結果,本論文證實在肝癌細胞中,YB-1會調控肝癌細胞的氧化磷酸化代謝途徑,而且會藉由改變此代謝途徑,進而影響肝細胞癌的發展。
