探討發炎反應與癌症發展之關係

在癌症發展之不同階段,從體細胞突變和不正常細胞增生至腫瘤生成與惡性轉移,發炎反應透過調控多重分子傳遞路徑、多樣的細胞行為和免疫反應來主導癌症之進程,說明發炎反應與癌症的發展息息相關。發炎反應主要分為兩種:急性發炎與慢性發炎反應。近年研究顯示,急性發炎反應對於癌症的發展多數具有抗癌之作用,然而其所扮演的真正角色,目前仍需更加深入研究。反之,慢性發炎已被證實與癌症的誘發以及後續的惡性轉化 (malignant transformation),還有腫瘤微環境 (tumor microenvironment, TME) 的生成有密切關聯。為研究發炎反應與癌症發展之關係,本研究主要深入探討腫瘤微環境中

的兩大重要組成:細胞外基質 (extracellular matrix, ECM) 與腫瘤相關巨噬細胞 (tumor-associated macrophages, TAMs) 對於非小細胞肺癌轉移之影響。此外,本研究亦探討急性發炎反應蛋白對於肝癌細胞分化與存活率的影響。我們首先針對細胞外基質的奈米形貌 (nanotopography) 開發具有序列性奈米粒徑的氧化鉭奈米點陣列 (tantalum pentoxide nanodot arrays) 作為體外人工微環境 (artificial microenvironment, AME) 以模擬細胞外基質如何誘導癌細胞轉移。透過所開發之體外人工

微環境,本平台成功誘導癌細胞轉移之不同階段的細胞進程 (cell transition) 與其相關的基因圖譜資料,並利用基因表現資訊找出相對應之潛力藥物,未來將有助於發展個人化藥物篩選與老藥新用。接下來,本研究發現腫瘤相關巨噬細胞與癌細胞所釋放的發炎蛋白YKL-40會受到非小細胞肺癌 (non-small cell lung cancer, NSCLC) 的表皮生長因子標靶藥物 (EGFR targeted therapy) 所誘發,且較高細胞外YKL-40表現量顯著地促進非小細胞肺癌之癌細胞的轉移 (migration) 與侵犯 (invasion) 能力。此現象說明標靶藥物的治療會引發YK

L-40表現量升高,進而導致治療所誘導之細胞轉移,最終造成治療效果不彰 (incomplete response) 與殘留疾病 (residual disease)。因此本研究更進一步開發YKL-40單株抗體以中和被誘發之YKL-40表現,可望作為現有治療方針的輔助用藥。最後,本研究透過肝癌臨床檢體與癌症基因組圖譜 (TCGA) 資料庫中的基因表現資料與檢體臨床資訊,證明急性發炎蛋白Haptoglobin (Hp)的表現量與肝癌細胞分化和五年存活率具有正相關性。換言之,Hp表現量越高,病人的肝癌細胞則分化的越好,且有較高的五年存活率。總結而言,為剖析發炎反應與癌症發展間的複雜關係,本研究透過腫

瘤微環境中細胞外基質與發炎蛋白YKL-40以及急性發炎蛋白Hp,以多方角度與機制切入探討不同因子如何調控癌症的發展,包含癌細胞轉移、治療效果、癌細胞分化程度與病人的五年存活率。更重要的是,本研究之研究成果具高度潛力應用於藥物篩選、癌症治療與臨床診斷,期望能有效輔助現今癌症治療上所面臨的困難與瓶頸。

The minimum distance approach has been developed and utilized for successful detection of possible collision obstacle and further study is carried out to generate point cloud environment of the

static obstacles and path planning in the work space of the robot. The characteristics of 3D point cloud appears to be same as of real environment.
