大學生不安全依附傾向對分手後自我概念清晰之效果研究:以自我擴張、自我驗證為中介變項

鑒於失戀為大學生常見的議題,以及伴隨而來的自我概念混淆和憂鬱皆是失戀時常帶來的困境,然而,親密關係對個人自我概念的改變以及失戀如何影響個人自我概念的相關研究甚少,且與親密關係息息相關的依附理論,在失戀影響自我概念的歷程中扮演的角色更是缺乏進一步的探討。因此,本研究結合「焦慮型依附」與「逃避型依附」兩種不安全依附類型,以及「自我擴張」、「自我驗證」兩種關係中的自我概念改變歷程,提出「不同依附傾向之自我概念改變歷程對分手後自我概念清晰度影響之模式」,探討以下議題:(1)「焦慮型依附」、「逃避型依附」對分手後「自我概念清晰」之預測效果;(2)「自我擴張」與「自我驗證」兩種自我概念改變歷程於前述預測

關係中的中介效果。為了檢驗本研究之模式,本研究採用網路問卷,測量大學生之依附傾向、關係中的自我擴張、自我驗證經驗以及分手後之自我概念清晰。參與者為336位大專院校學生(男性111人,女性225人)。驗證性因素分析與信度分析結果顯示,本研究各分量表之信效度皆可接受。結構方程模式分析結果顯示:(1)「焦慮型依附」負向預測「自我擴張」與分手後「自我概念清晰」;(2)「逃避型依附」負向預測「自我驗證」;(3)「自我擴張」負向預測分手後「自我概念清晰」。本研究之結果對於親密關係失落的理論或實務皆有參考價值。

研究背景全球第二型糖尿病的盛行率逐年攀升,而台灣糖尿病盛行率於2019年統計也已經超過11%,且絕大多數為第二型糖尿病,高血糖問題儼然成為我國國人健康的最大隱憂。糖尿病為一慢性全身性疾病,若長期控制不好會導致諸多併發症的產生,研究顯示,初發病的糖尿病病人若積極治療能顯著降低發生大小血管併發症的機率與降低死亡率。早期基礎胰島素介入治療可使病人血糖快速趨於正常水平,減少不可逆併發症的發生,為病人帶來長期的好處。然而大部分第二型糖尿病患者對於注射胰島素仍抱持抗拒的心態,因而耽誤使用胰島素治療的時機。國外對於心因性胰島素阻抗已有多項研究,台灣的胰島素注射率遠低於歐美國家,卻鮮少有研究深入探討台灣民眾

對於使用胰島素控制血糖的看法,因此我們進行這項研究,希望透過問卷方式了解糖尿病患者對於使用胰島素治療的態度。本研究分別以計畫行為理論和科技接受模型為理論框架,發展出新的量表(以下簡稱為計畫行為理論量表和科技接受模型量表),以了解哪些因素會影響糖尿病患者使用胰島素的行為意圖,並進一步比較兩個理論何者較適合用於探討施打胰島素的行為意圖,進而將研究結果應用於衛教上,提升胰島素注射率和糖尿病照護的成效。研究方法本研究為一橫斷性研究,採匿名式問卷,同時於板橋某家醫科診所和網路進行問卷調查。問卷由計畫行為理論量表和科技接受模型量表所組成,其內容包含人口統計資料、糖尿病及其治療相關的知識測驗及影響病患使用胰

島素行為意圖等題項。問卷首先聘請專家做內容效度評估,使用條目水平的內容效度指數(item-level CVI)作為刪題標準,接著以便利性抽樣選取13位第二型糖尿病患者進行前驅測試,二度確保問卷內容的適切性,待問卷全數回收後再執行項目分析、建構效度分析和內在信度分析,以確立問卷的信效度。確保問卷結果符合信效度要求後,利用潛在變數路徑分析法(path analysis with latent variables, PA-LV)分別驗證計畫行為理論研究假設中,態度、主觀規範、知覺行為控制、知識對行為意圖的影響,與科技接受模型理論研究假設中,知覺易用性、知覺有用性、使用態度、外部變數對行為意圖的影響。

最後以行為意圖的解釋變異量比較兩個模型解釋力的優劣。研究統計分析p值皆為雙尾檢定,並定義p值小於0.05具有統計學上顯著差異。研究結果計畫行為理論量表由態度、主觀規範、知覺行為控制、行為意圖4個分量表組成,經項目分析、探索性因素分析、驗證性因素分析與信度分析後,總題項數由36題刪減成28題,各分量表信度指標α係數皆>0.6,總量表α係數=0.915,具良好信度。探索性因素分析將態度分量表和知覺行為控制分量表題目各自分為2個構面,符合計畫行為理論原設計信念,唯主觀規範分量表題目分成的2個構面與原設計信念不符,所有題項之因素負荷量皆>0.5。階層驗證性因素分析整體模型適配度指標X2/df=3.04

1、GFI=0.86、RMSEA=0.072具良好適配度,CFI=0.891、TLI=0.876為可接受適配,顯示模型仍有改善空間。計畫行為理論量表對行為意圖的解釋力為83%,其路徑分析結果顯示,態度、主觀規範和知覺行為控制皆會正向直接影響行為意圖,知識則會透過態度和知覺行為控制間接影響行為意圖。科技接受模型量表經項目分析、探索性因素分析、驗證性因素分析與信度分析後,總題項數由15題刪減成13題,量表信度指標α係數=0.905,具良好信度。探索性因素分析將題目分成4個構念,符合科技接受模型理論,且每個構念題項數都保留3項以上。驗證性因素分析整體模型適配度指標為X2/df=2.561、GFI=0

.947、RMSEA=0.063、GFI=0.969、TLI=0.959,表示模型適配度優良。科技接受模型量表對行為意圖的解釋力為67.2%,其路徑分析結果顯示,知覺有用性和知覺易用性皆會正向直接影響對胰島素的使用態度,使用態度是行為意圖的主要決定因素,知識則會透過知覺有用性對行為意圖產生間接影響效果。結論計畫行為理論量表和科技接受模型量表皆能良好預測行為意圖,但計畫行為理論對行為意圖的解釋力更高,且能提供更多的資訊,故本研究認為,計畫行為理論較科技接受模型更適合用來預測和解釋第二型糖尿病患者施打胰島素的行為意圖。而根據計畫行為理論量表的結果,施打胰島素的態度為第二型糖尿病患者施打胰島素的行為

意圖之主要決定因素,態度又會受到知識的正向直接影響,故醫療人員可透過加強民眾對於胰島素的正確知識來改變病患對施打胰島素的態度,提供糖尿病患者最適當的藥物治療。最後我們可將本研究結果應用於臨床上,提升第二型糖尿病患者使用胰島素的意願,以改善糖尿病治療的整體照護成效。
