我國長期照顧計畫邁入第二個10年計畫，長照2.0於2017年實施，政策以延緩失能、失智、減少醫療支出、緩解家屬照顧壓力為目標，盼能獲得更完善且有尊嚴的人性照顧。2019年長照推出「居家失能個案家庭醫師照護方案」，結合家庭醫師制度，建構居家失能個案在長照與醫療整合之照護網絡。由醫師每半年開立一次醫師意見書，提出長照醫事照護意見，目標為提升在宅照護連續性，落實在家安老。社區診所非以居家醫療為主，本研究為能了解診所醫師在初步介入長照服務領域之動機與實施過程，探索到宅開立醫師意見書的影響因素，故本研究透過質性研究方法，採用立意取樣，共深度訪談5名新竹市基層診所之醫師，藉由醫師實地投身長照服務的豐富經

驗，探討目前基層診所投入我國長期照顧服務之後所產生的改變與如何發揮整合性的功能。研究結果發現，基層診所要與居家失能個案家庭醫師照護方案特約及提供長照服務前，需先完成長照人員Level-I 18小時基礎課程，並能熟悉長期照顧醫師意見書表單內容，方能順利完成到宅服務。同時，基層診所醫師只能利用休診時間，進行長照個案到宅服務，因此每週工時受限，影響可到宅服務的長照個案數量能。長照服務申報系統與診所慣用醫療作業系統不同，每次到宅開立長期照顧醫師意見則需耗時準備與耗工整理個案資料。再者，醫師在服務長照的個案中多非診所的病人，與長照個案建立之醫病關係需要耗費更多的時間方能建立信賴關係，需長時間之投入。研究

建議政府單位可與診所協會、醫師公會合作，分梯辦理實體與線上長照專業課程，平時工作非常忙碌的診所醫師或護理師可以利用閒暇時間進修。建議開放診所共享健保醫療資訊雲端查詢系統，才能讓到宅提供專業醫事服務的工作人員落實健康管理及疾病衛教。當基層診所人力不足時，可選擇與居家護理所共同合作來管理長照個案，減輕行政作業程序之負擔。2021年5月因為COVID-19疫情爆發，加上國人防疫觀念的提升，部分專科診所就診人數大幅減少，建議社區診所若能有志投入長照服務的工作，不但可提升診所知名度、增加診所新收案人數，也可讓診所有穩定收入來源。兩名受訪者建議提高報酬，或是有假日/夜間加計給付制度，才能讓基層診所醫師投身

長照服務之意願可以更長久。
