非結核分枝桿菌 (Nontuberculous mycobacterium,NTM) 被認為是環境污染菌株之一。然而,近年來由其造成的感染性疾病發生率顯著增加,如: 肺部感染、軟組織及傷口感染、淋巴腺感染以及造成全身性散佈感染等,因此,非結核分枝桿菌在近年來逐漸受到重視。而在台灣目前最盛行的則為Mycobacterium avium complex (MAC)、M. kansasii 以及 M. abscessus complex,且因 NTM 種類繁多,其生長速率、溫度耐受性和藥物敏感性方面差異很大,使得由 NTM 引起的感染通常難以治療且 NTM 容易轉換成為抗藥性菌株,多半需同時結合多

種抗生素使用,因此新一代抗生素的研發及尋找抗生素的替代品更顯迫切需要。在本研究中,首先主要篩選 20種小檗鹼衍生物 (Berberine derivatives) 對於常見病原菌以及 NTM 的抗菌活性。希望能尋找出新的小檗鹼衍生物能有較低的抗菌MIC濃度及低的細胞毒性,先以挑選常見的革蘭氏陽性 (Gram positive) 細菌、革蘭氏陰性(Gram negative)細菌及非結核性分枝桿菌(Nontuberculous mycobacteria,NTM) 標準菌種進行測試。篩選實驗方法以紙碇擴散試驗 (Disc diffusion assay) 進行,其實驗結果發現其中4種小檗鹼衍生物

(B3、B4、B11和B12) 對常見病原菌以及 NTM 有顯著抑制效果。更進一步評估4種小檗鹼衍生物的抗菌效果,則以最小抑菌濃度 (Minimum inhibitory concentration,MIC)進行評估。結果顯示4種小檗鹼衍生物 MIC 約在1〜256 µg/mL比小檗鹼約在16〜512 µg/mL有更好的抗菌效果。並在小檗鹼及小檗鹼衍生物進行 MIC 時,在培養4小時後以波長 420 nm (5.6mW/cm2) 照光10分鐘,看照光前後對 MIC濃度的影響,實驗結果顯示照光與未照光對於 MIC 沒有任何變化。另外為了了解小檗鹼及其衍生物與治療NTM的最重要藥物之一的Amika

cin是否具有協同或拮抗作用,以部分抑菌濃度指數 (Fractional inhibitory concentration,FIC) 的實驗進行評估,結果顯示小檗鹼及其衍生物與Amikacin對M. chelonae (ATCC35752) FIC index < 0.5有協同作用,但對M. abscessus (ATCC19977) FIC index 1.25無差異性。最後測試具濳力之小檗鹼衍生物的細胞毒性,以人類肺癌細胞A549測試不同濃度的小檗鹼及其衍生物,實驗結果發現小檗鹼毒性不高,但小檗鹼衍生物B12 對 A549 毒性較高。綜合以上結果,新的小檗鹼衍生物可做為治療 NTM 新的藥

物,或是合併臨床藥物藉以提高治療 NTM 效果。
